I would like to thank you for the fruitful evening you hosted on the 4th of April.
As discussed in our presentations, the “Frailty Hub” at SVP is an initiative to create a base where frailty management and research on the island will be lead. Through the past six years, we took part in European activities for frailty management excellency, nevertheless we conducted our own research through the frailty working group in Malta.
As you all know our Geriatric services are challenged with a heavy need for solutions for the elderly living with frailty in the community. These elderly are the ones who are easily r
deconditioned when exposed to mild health challenges, making them in need for acute hospitalisation and unfortunately, frequently in need for institutionalisation.
Frailty presents a global challenge to the active ageing initiatives, hence our cooperation is needed in order to detect it at early stage and manage it/ revert it whenever possible.
I am cordially inviting you to be our partners in this initiative by helping us to spot the clients in need for immediate assessment and action.
Kindly find attached a small leaflet being distributed to raise the awareness of this subtle condition which carries unpleasant consequences and effects on the welfare of our patients:- Frailty clinic
You will all receive this leaflet in a hard copy in due time.
We are gladly ready to answer any query and continue to familiarise our colleagues in the primary care sector with means to suspect this condition in their client cohort.
The referral for the frailty clinic can be made through the CTOR system.
Currently we are holding a weekly clinic and we are planning to expand it according to needs and capacity.
