In light of the ongoing changes within the Podiatry Biomechanics Services within Primary Health Care, we have introduced a new online referral form to streamline the process for all. Please note that this will now be the sole accepted method for submitting referrals, in order to avoid any confusion.

The referring clinician is required to complete and submit the form, which will be received by our team promptly. Following submission, patients will be triaged by our colleagues in the Podiatry Telemedicine Team, who will then contact patients directly to arrange an appointment. As a result, no appointments should be booked by the clinician or by the patient.

Should you encounter any issues or have any suggestions for improvement, please do not hesitate to let us know.
