A few weeks ago the new website of the Health Promotion and Disease Prevention Directorate (HPDPD) was launched.  This Infectious Disease Prevention and Control Unit (IDCU) page has been updated, including a new page with access to on-line notification forms.

Since the addresses for the relevant pages have now changed we are sending below the links that you can bookmark to be able to access them with ease:

Should you want to search for the page via a browser you need to type in “notification form hpdp”.
