Domiciliary BP Monitoring

Dear all,

The Active Ageing and Community Care is strengthening its domiciliary nursing blood pressure monitoring services in order to provide maximum benefit to elderly patients. The service is specifically tailored for home-bound residents who are unable to attend health centres or primary healthcare services at community clinics.

Attached please find information regarding this service as well as the referral form through which this service can be applied for. I invite you to circulate these amongst family doctors and doctors under your remit.
