WHO supports countries to implement the integrated care for older people approach (ICOPE) focusing on optimizing people’s intrinsic capacity and functional ability as they age and on the delivery of a continuum of integrated care. The ICOPE handbook: Guidance on person-centred assessment and pathways in primary care, provides a resource to support health workers to put recommendations into practice in primary care including the community.
The update of the ICOPE handbook (second edition) was informed by the experience and learning from around the world in different resource settings and incorporates latest WHO recommendations across a range of technical areas. Acknowledging the vital role of community, greater attention is given to the role of community, especially the expanded first step in the care pathway. The earlier identification of social support needs and carers support needs and new care pathway for management of urinary incontinence are included.
The launch event will include a presentation of key changes in the second edition and panel discussions with community stakeholders and health workers in primary care. It will also highlight older people’s perspectives of integrated care.
