The referrer may log in either via CORP account or his/her E-ID.

The updated hard copy that a General Practitioner/Family Doctore may fill, sign, scan and submit via e-mail on transhealthcare.health@gov.mt​​  can be found here:  Referral form to the Gender Wellbeing Clinic  .  Any queries may also be sent to this email address.

Please note that the Gender Wellbeing Clinic now runs its service from A3 Towers, Arcade Street, Paola.  All clients are seen by appointment.
